Big April Update: Incident Reports, Inspection Packs, and a Brand-New Wizard

We just shipped our biggest release of the year. AI-drafted incident reports, six industry inspection packs, a completely retooled SWMS wizard, and a stack of security upgrades — all in one go.

Big week. We've been heads-down for the last few sprints, and today we're rolling out the largest AxionSite update since launch. Three brand-new modules, a completely rebuilt SWMS wizard, a hardened security stack, and a refreshed help centre — all live now.

Here's the quick tour.

1. AI-drafted incident reports

If something goes wrong on site, the last thing your team wants to do is fight a 14-page form. So we built a single, voice-first flow that turns a short account of what happened into a complete incident record.

How it works:

  • Open Dashboard → Incidents → Report incident
  • Dictate (or type) a plain-language description: *"Fitter struck on left hand by swinging load during crane lift at Bay 3, reported to supervisor, sent for first aid."*
  • Fill in the basics — when, where, severity, category
  • Hit Draft report with AI and you get a full pack back in seconds
  • Every AI draft comes with:

  • A clean narrative you can paste straight into your IMS
  • The sequence of events reconstructed from your account
  • Contributing factors and likely root causes
  • Immediate and longer-term controls
  • A clear notifiable flag mapped to WHS Act s.35–s.39 so you know whether you need to call the regulator
  • Suggested corrective actions with owners and due dates
  • Witnesses, injuries, and photo captions are all optional — add as little or as much as you need. Every record exports to a branded, audit-ready PDF with the full legal disclaimer baked in.

    And because regulators love an audit trail, nothing ever gets hard-deleted — status changes and soft-deletes are tracked per record, immutably.

    2. Inspection / checklist packs (six templates built-in)

    This one is for the teams who've been asking for "something like Dashpivot, but without leaving AxionSite." Done.

    We've shipped six pre-built inspection templates based on Australian Standards and industry codes:

  • MEWP pre-start check (AS 2550.10)
  • Confined space entry permit
  • Lock-out / tag-out (LOTO) verification
  • Mobile scaffold inspection (AS/NZS 1576)
  • Fall-arrest harness pre-use inspection (AS/NZS 1891)
  • First-aid kit monthly audit
  • Each template is a real checklist, not a PDF dump. You pick the template, fill in the site and inspector details, and then walk through each item on mobile — Pass / Fail / N/A, with optional notes and a corrective action field for any fails. Critical-fail items are called out in red so they can't be missed. A live progress bar keeps you honest.

    Two things we care deeply about here:

  • Template snapshots. — When you start an inspection, the template is frozen onto the record. If we update the template next month, your closed inspection from this week stays exactly as it was — critical for auditability.
  • Signed PDFs. — Every completed inspection exports with inspector, supervisor, and compliance-statement signature blocks, plus the AS/NZS reference at the bottom.
  • Start an inspection from Dashboard → Inspections → Start inspection.

    3. A rebuilt SWMS wizard that actually feels good on a phone

    The biggest design work this quarter went into the SWMS creation flow. We tore it apart, audited it against SafetyCulture and Sitemate, and rebuilt the UX from the ground up for 2026.

    What's different:

  • One progress indicator per screen. Numbered stepper on desktop, a clean linear bar on mobile — never both, never competing.
  • All primary buttons are touch-friendly. 44px minimum tap target, safe-area bottom padding so the "Next" button never hides under the iPhone home indicator.
  • No more shrunk text on mobile. Every input is at least 16px so iOS doesn't zoom in when you tap it.
  • Inputs never disable — they show or hide. Less guessing about why a field won't respond.
  • Bottom-sheet wizard on narrow viewports. On a phone the wizard now slides up from the bottom like a native iOS sheet, with a grab handle and a proper safe area.
  • Start-from-recent and resume-draft. If you've already generated a similar SWMS, reuse it as a starting point in one tap. If you closed the tab mid-wizard, the draft is still there when you come back.
  • First-time users now reach their first generated SWMS roughly twice as fast. Returning users can get from "new task description" to "downloaded PDF" in under two minutes.

    4. Security hardening

    We did a full security audit and shipped fixes across the board. The bits worth calling out:

  • Strict Content Security Policy shipped via middleware — closes off XSS, clickjacking, and form-hijack vectors that were previously only partially covered.
  • Rate limits on the public contact form. 5 requests per hour, 20 per day, per IP. Applied before any backend call so it holds even if a downstream service is misconfigured.
  • PNG magic-byte validation on sign-on signatures. We now verify the first 8 bytes of every uploaded signature match the PNG spec (`89 50 4E 47 0D 0A 1A 0A`) — no more accepting anything that merely claims to be a PNG.
  • Cross-Origin-Opener-Policy added to the baseline headers for proper isolation of OAuth popups.
  • Storage rules tightened alongside the new incident and inspection collections.
  • None of these break existing flows — they just quietly make the platform harder to attack.

    5. A better help centre

    We replaced the old "Jump to" nav on the in-app help page with a proper search panel, and added dedicated topic entries for the new wizard, incident reporting, and inspection packs. Search the keyword, jump to the walkthrough, and get back to work. Two new "What's new" cards announce the big modules right at the top.

    How we compare now

    With incidents and inspections in the box, AxionSite now covers the three core EHS flows that used to require stitching together two or three SaaS tools. Here's the updated picture:

  • Enterprise SSO / SAML (we scoped it; it's the next big rock)
